Senior College Information Form Available Online
Following the Seniors’ and Parents’ Mandatory Meeting on Monday, April 16, the College Information Form is now available online. The form allows graduating students to submit information about college application and acceptance, scholarships received, and intended majors. Jesuit will use the information for its publications surrounding commencement. The form is also the means by which students request transcripts to be sent to their chosen school.
Seniors are asked complete one form per student. Once the form is submitted, the confirmation email should be printed and signed by the student and a parent/guardian. Students will also need to attach copies of scholarship awards to the form and return it to the Counseling Office by Monday, April 30, 2018.
Should any information change after the form is completed, seniors must notify their college planning counselor in person. If accepted to any schools or receive any additional scholarship offers after completing the form, students should not attempt to fill out a duplicate form online, but bring the new information directly to their college planning counselor.
