Physicals for Jesuit Athletes 

 

The State of Louisiana requires that student athletes have a complete physical examination. According to the Louisiana High School Athletic Association Handbook (Section 16—Medical Examination):

 

“In order to be eligible for practice or participation in all interscholastic athletic contests, a student must receive and pass a medical exam prior to the sport season or at least once every 365 days by a physician licensed to practice medicine.  A medical exam form must be on file in the school office of every student who practices or participates in interscholastic athletics during that school year.”

  

You will find below a link to the physical examination form that is used by Jesuit High School.

 

The athletic participation/ parental permission form, the substance abuse form, and the football informed consent form are also posted. Parents are asked to read and sign these forms if their son is to tryout for or participate in any athletic team. These completed forms should be turned in to the coach of that respective team.
